Job Summary
The Triage LPN will review clinician documentation and work with the Director, clinical staff, and office staff to support workflow functions that impact quality of care, chart review, and timeliness of billing. The Home Health and Hospice Triage LPN provides routine direct and indirect patient care in the home that reflects the home health and hospice agency philosophy and standards of home care, under the supervision of a Registered Nurse and in accordance with ANA standards and NCBON. As well as triage calls from physicians’ offices, hospitals, referral sources, co-workers, and patients/families, etc. The Triage LPN will also need to assist with patient visits as needed. The Home Health & Hospice Triage LPN provides services in the home or other appropriate dwelling to home health and hospice patients as designated in their plan of care. They provide services that reflect the Mission, standards, and philosophy of the Hot Springs Health Program.
Essential Duties
A. Maintain PT/INR flow sheets and results from RN Case Managers.
B. Manage incoming lab results and any follow-up if needed with ordering Providers.
C. Will assist with the quality assurance and improvement activities for the agency. Look for ways to improve and promote quality.
D. and inform the home care director of any deficiencies.
E. Perform insurance authorizations, submission of documentation, and verification.
F. Perform scheduling of starts for all disciplines.
G. Adhere to the practice of confidentiality regarding patients, staff, and organization.
H. Monitor agency outcomes on specific process measures, as reported by CMS, and assist with providing education to clinical staff on areas with deficiencies.
I. Will assist the Director with the timely review of staff visits in EMR to ensure smooth and continuous billing workflow.
J. Will assist the Director with following all OSHA, HIPAA, and safety procedures. Abide by all federal, state, and local regulations.
K. Other duties as assigned.

F. Maintains licensure and appropriate clinical skills.
G. Prioritizes and plans work activities and patient care.
H. Timely completion of referrals, patient assessments, and care plans when needed.
I. Ensure quality and safe delivery of home care services.
J. Gather necessary and accurate information about patients.
K. Perform technical skills and procedures as ordered by physicians and supervised by an RN.
L. Communicate with co-workers, patients, family, and other ancillary departments.
M. Accurately and timely, complete all documentation.
N. Accept responsibility for personal growth/Accountability/Time Management.
O. Provide respite and/or continuous care for hospice patients.
Education and Experience Requirements
A. Education: Educational preparation with licensure to practice in the field of professional nursing in the state of North Carolina. Graduate of an accredited LPN or program. Must have a current North Carolina LPN license.
B. Experience: Minimum of 2 years of home health/EMR and OASIS experience preferred
C. Other: Valid N.C. Driver’s License, proof of auto liability insurance, and dependable transportation
